Applications and Requirements
You love your pets. Chances are you spoil them–taking them to dog parks, lakes, daycare and any other place to show them a good time! Pets are part of the family. These days they can be exposed to many different types of viruses and bacteria due to many factors.
That’s why Dogtown Cincinnati requires pet owners keep their pets vaccinated and flea-free, and bring them in clean (or with a grooming appointment.) We work hard to keep our daycare home-like and clean, so we will not accept filthy dogs without an appointment for a bath!
Applications for Services
Below is everything you need to get started with services at
Dogtown Cincinnati
ENROLLMENT FORMS (REQUIRED):
1. You will need to read and/or fill out all of the forms below when you come in, in addition to proof of the required vaccinations as stated on the forms below:
> Liability Waiver and Health Certification
> Services Welcome Cover Letter
> Family General Enrollment Application
> Health Requirements (for your vet) WE WILL CONTACT YOUR VET IF YOU NEED US TO!
2. You will need to fill out one personality profile for each pet in your family that will use Dogtown Cincinnati's Services:
> Dog Info and Personality Profile (fill out one per dog in the family)
> Cat Info and Personality Profile (fill out one per cat in the family)
DAYCARE MEMBERSHIP ENROLLMENT FORMS (OPTIONAL):
3. You will need to read and fill two PDFs in order to enroll your pet(s) into a daycare membership regimen. Memberships are optional. A regular schedule is required. Click here for more information about memberships
